The Smile Package involves 2 stages:

Stage 1 prepares you for the big case presentation you have in mind for the patient. This includes uploading the files needed onto the Hub, a quick questionnaire regarding the treatment plan and finally presenting the case with the model, wax-up, shell temps, and a custom powerpoint presentation made for you.

Stage 2 is the final part of the workflow that involves teeth prepping, assessment and the final restorations.

What is included in the smile package:

• Powerpoint presentation

• Design files for:

  • Model

  • Diagnostic wax up

  • Shell temps

  • Final restorations

What you need to get started:

• Natural smile photo of patient (see Smile Mockup for instructions on how to take photos)

• Clean STLs:

  • Maxillary scan

  • Mandibular scan

  • Bite scan

• Questionnaire (must fill out before design starts)

How to order the Smile Package

Stage 1

1. Sign in to your Hub account (and create a new patient if you haven’t already)

2. Click the patient you are ordering a smile package for and go to the Treatment Cases tab

3. Click Submit New Case and select the following

   • Type of service output: Design

   • Type of treatment: Other – Smile Analysis and Planning Package

   • Service provider: Evident Design Services

4. Attach the clean STL file and natural smile photo of the patient

5. Your dedicated account manager will be in touch shortly to fill out the questionnaire before beginning the design process

6. Once the questionnaire is filled, the design process starts and you will receive:

   • Powerpoint

   • Design files for:

     • Shell temps

     • Model

     • Optional snap on

7. Present the case to your patient!

Stage 2

1. After the patient approves the case, prep the teeth/use shell temps for try-in and assessment of accuracy for the designs

2. We will need a new scans of the following:

   • New preps

   • New bite

   • New temps

3. Upload the new files to the Hub and you will receive the design files for the restorations to give to your patient within X days? (Is this step finalized?)
